When your child is hospitalized or confined to home for medical reasons (such as physical, emotional or behavioral situations), it is important to contact your child's school counselor immediately so they may continue to support your child and your family.
Once you have signed a release form, the school counselor will assist you in communicating with the hospital point of contact or the physician regarding your child's progress, ability to do schoolwork and planning for re-entry back to school.
Once discharged or return to school is discussed with the hospital treatment team, the school counselor will assist in setting up a re-entry meeting for you and your child. It will involve many people who will provide support to your child upon school re-entry. It is never a good idea to just send your child back to school without a re-entry meeting. For your child's safety and sense of well-being, we all need to know our roles and responsibilities.
The meeting will allow us to plan for the student to return to school with no demands, be greeted by familiar adult support, review discharge information as well as be a part of developing the best plan for a smooth transition. The plan will include social / emotional needs and academic make up work.
